    By signing this application, I agree to the following terms and conditions: 
    • Tuition must be paid in advance by the first day of each month.
    • No Credit and makeup are given for unexcused absences.
    • Vacations are allowed with prior notification and a make-up will be planed.
    • Notification of inability to attend a lesson does not excuse payment for the session.
    • If the student cancels in time or if the instructor has to cancel the lesson, a make-up will be offered.
    • Make up lessons cancelled by student will not be offered again.
    • Tuition will be non refundable, non changeable and non transferable.
    • Lessons will take place at the River Forest Community Center located at: 8020 Madison St. River Forest, or 1535 Park Ave. River Forest, or 1514 Lathrop Ave. River Forest or online lesson also available.
    • Participants assume full liability and responsibility for damage or loss of her/his instrument.
    • The SCO School of Music is  not  responsible for any personal injuries that might happen during section practice and performance times.
    • Tuition rate per 30/60min individual Lesson:  $45/$90 Beginner Level, $50/$100 Intermediary Level, $55/$110 Advance Level, Intensive Lessons for College auditions and National competitions are available.  
    • $25 application fee (also due every time a student quits and restarts their session) 
    • No deposit requests and No application fees. Payment by Cash, Zelle Quick Pay (773) 203 2816, Check is payable to the Sunflower Chamber Orchestra School of Music or directly to your teacher.
    • Mandatory Performance Activities: Students will perform several times throughout the year. Those performances include student recitals, workshops, studio classes, ensemble groups and student competitions, these additional activities will generally incur some nominal fees to cover expenses relating to the activities.
    • I agree that SCO School of Music may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
