Financial Assistance Application for SCO only

The Sunflower Chamber Orchestra (SCO) allows all young musicians from Chicago and the suburbs to have an equal opportunity by offering a financial assistance program to reach their dreams in performing. The SCO believes in the unlimited energy and power of the young generation and the ability of the family to pay is not a reason to hide the talented kids.

Financial Assistance is simple and based on a family’s income. TheSCO Board will determine the number of applicants and the amounts of given availability of donations.

The SCO’s Financial Assistance program will work hard to satisfy all applicants’ needs and will offer a scholarship if needed or if possible. Please fill out form bellow and submit. The SCO Board will review your application and get back to you promptly.

NOTE: Upon review from our chamber board, a copy of first page of your 1041, federal income tax form showing adjusted gross income is required as proof of income. Please be prepared to submit a physical copy to our music director upon request. Please make sure to block out your Social Number, the SCO doesn’t use it. Application that missed this information will be not considered.
